Penobscot Community Health Center will hold a COVID-19 vaccine clinic at Mattanawcook Junior High School on Friday, May 21st. All students who are 12 years old and above on that date are eligible. The clinic starts at 9:00 a.m. PCHC will return in three weeks to administer the final dose of the Pfizer vaccine. You can print and complete this permission form or pick up a printed copy in the office. Completed forms are due May 19th. Any remote students who are arriving by car and not in quarantine should plan to park in the teacher's parking lot or by the rocks at the end of the softball field and walk over to the tents.  Please do not park along the sidewalk by the softball field or in the bus loading zone in front of the building because these spots will be used by students in quarantine who must remain in their vehicles.

If you have a student in quarantine, they may still attend the clinic as long as they remain in a vehicle. Please drive along the sidewalk along the softball field like you would for student drop-off. We ask that the person transporting the student come to the tent to advise staff that the student is present. If the quarantined student is driving themselves and a staff person does not approach the vehicle, please call the school office at 794-8935 to notify us of your arrival. PCHC will administer the vaccine in the vehicle. Be prepared to wait in the vehicle for 15 minutes following the vaccine administration. Completed vaccine forms can be emailed to jdavis@rsu67.org or jmckechnie@rsu67.org or faxed to MA (794-3205) or MJHS (794-2601).
