Covid-19 Vaccination Clinic for Ages 12+ 

Penobscot Community Health Center will be holding a COVID-19 vaccine clinic at Mattanawcook Junior High School on May 21st beginning at 9:00 am. The clinic is open to all MA students and Mattanawcook Junior High students who are 12 years old and above,  and have not already received a COVID-19 vaccine elsewhere. Transportation for MA students will be provided.  Parents and Guardians of MA students will receive an email with the permission slip attached.  MJHS students will be bringing permission forms home with them. If you did not receive an email or a form, permission slips will be available for pickup by students or families at both the MA and MJHS offices. PCHC will be returning on June 11th to administer the second dose of the Pfizer vaccine. 

You can print and complete this permission form or pick up a printed copy in the office. Completed forms are due May 19th.
